Our work spans statewide network development, shared data infrastructure and direct program delivery support, from initial partner onboarding through referral pathways, training and long-term sustainability planning. We’ve built this expertise through sustained funding relationships with the CDC and the Administration for Community Living — which require the same discipline any state or community partner needs: proving program value, managing multi-partner networks and adapting infrastructure as funding cycles shift. Recent engagements

Sustaining a statewide chronic disease network across three states

We built and now operate the Community Integrated Network of Oregon and the Wellness Integrated Network of Nevada, engaging more than 70 partners and supporting over 22,500 program enrollments through our Compass Data Portal.

Reaching underserved communities with diabetes prevention

As a CDC 1705 recipient, we helped 13 affiliates establish CDC-recognized National Diabetes Prevention Programs in underserved areas, enrolling more than 2,000 participants.

Building culturally responsive diabetes programming

We developed Spanish-language diabetes self-management programming and Promotora de Salud training, connecting Spanish-speaking communities to diabetes prevention and management resources.
