Referral form
Beginning June 22, 2026, providers, self or member, family, friend or other will use one standardized referral process to connect Alaska Medicaid members to case management and care coordination services. This approach simplifies referrals, reduces administrative burden and ensures members are connected to the appropriate support without delay. Please note the reasons for referral include non-medical assistance (e.g., New Medicaid enrollee, navigating benefits, financial and/or social support).
