Overview

The Alaska Medicaid Coordinated Care Initiative (AMCCI) is designed to assist Alaska Medicaid members who need help navigating the health care system and can benefit from case management services. Because these members may have trouble navigating the health care system, they may become frequent users of the hospital emergency room for issues that may be better treated in a primary care setting.

Beginning June 22, 2026, providers, self or member, family, friend or other will use one standardized referral process to connect Alaska Medicaid members to case management and care coordination services. This approach simplifies referrals, reduces administrative burden and ensures members are connected to the appropriate support without delay. Please note the reasons for referral include non-medical assistance (e.g., New Medicaid enrollee, navigating benefits, financial and/or social support). Find the form on either the Providers or Patients and Families web pages.

The Division of Health Care Services has contracted with Comagine Health to provide case management services to high-utilizing AMCCI members. Comagine Health works closely with the medical and social service community to support and help coordinate care for these patients.

The AMCCI is a voluntary program.

Goals of the AMCCI

The primary objective of the AMCCI is to promote quality, cost-effective health outcomes by ensuring that coordinated medical services are provided in the most appropriate setting. Other goals include focusing on prevention and enhanced integration of primary medical care and behavioral health services. And most importantly, the AMCCI hopes to make a difference by improving the health and quality of life for Alaska Medicaid members.
