Conditions for Coverage and Conditions of Participation
ESRD Facility Conditions for Coverage are in the Code of Federal Regulations (dialysis regulations are in 42 CFR 405, 410, 413, 414, 488 and 494). Transplant facility Conditions of Participation are in 42 CFR 405, 482, 488 and 498. Adherence to these Conditions is monitored by Survey & Certification personnel from state health departments.
