Wait for Network notification to begin EQRS/QIP access
- Register for a HARP account (new EQRS users)
- Register for EQRS access
Complete the Memorandum of Understanding (MOU) with pending CCN — provided by the Network
Update EQRS with all key personnel Access facility personnel definitions
Keep the Network updated on your facility’s certification status
After CMS certification
Submit the following documents to the Network as soon as you receive your CMS Certification letter:
- Submit a copy of the CMS Certification letter
- Complete the Memorandum of Understanding (MOU) with updated CCN — provided by the Network
- Submit updated New Facility Details Form (if changes are needed from initial submission)
Update EQRS with all key personnel Access facility personnel definitions

Use the appropriate steps below to report and manage facility changes.
These forms must be submitted to the Network whenever changes occur. All changes must also be entered into EQRS.
- Facility Personnel Definitions and UpdatesDefines required personnel roles and responsibilities and provides instructions for updating personnel in EQRS.
- Facility Information Update FormNotifies the Network of significant facility changes (e.g., services, stations, ownership). Submit this form when changes occur.
- Memorandum of Understanding (MOU)Agreement between facility and Network. Complete the MOU when provided by the Network.
Only provide information for the fields where a change has occurred.
