Community Integrated Network of Oregon

Overview

The Community Integrated Network of Oregon (CINO) is a network of diverse partners building statewide infrastructure to deliver and sustain evidence-based health education programs and interventions. CINO is led by Comagine Health and Oregon Wellness Network (OWN).

In Oregon, six of the 10 leading causes of death are chronic conditions. CINO is increasing access to community-based services and supports that:

  • Help people prevent and manage chronic conditions, including heart disease, stroke, chronic lower respiratory disease, diabetes, chronic liver disease/cirrhosis and hypertension
  • Reduce preventable hospital and emergency room utilization
  • Improve health for Oregonians

CINO is a scalable model for providing equitable, accessible, inclusive health education and support to all Oregonians.

Our services

Program Delivery — CINO supports program delivery for evidence-based health education programs and interventions. Our program offerings are available on the Oregon Compass self-management portal. We provide orientation and onboarding support for program coordinators and organizations new to program delivery as well as an array of assistance for organizations with existing infrastructure.

Referrals — We support a statewide referral hub for health education programs and interventions. There are multiple ways to refer to the statewide referral hub. We maintain connection to Connect Oregon for referrals coordination and management at the state level, screen referred participants and provide program enrollment through the Oregon Compass self-management portal.

Reimbursement — CINO supports reimbursement for partners by maintaining the accreditation and recognition standards required to bill for some evidence-based health education programs. We contract with vendors for claims and invoice submissions as well as with health plans and payers. At the state level, CINO bills on behalf of program delivery organizations and distributes payments to them.

Convening and Technical Assistance — We convene partners and provide technical assistance with staffing and facilitation for the CINO Operations Steering Committee and three supporting subcommittees focused on referrals, sustainability, and marketing and engagement.

Infrastructure Development and Design — CINO coordinates with national and local experts to design and develop the statewide network. This includes planning trainings and skill development opportunities for program coordinators and leaders. We contract with experts to provide technical assistance and culturally specific, accessible and inclusive programming.

Quality Improvement and Data Management — CINO aids in data management, oversight and technical assistance through the Oregon Compass self-management portal. In addition, we monitor program quality, provide guidance to program delivery partners and address concerns.
